- PRICE (Protection, Rest, Ice, Compression, Elevation)…
PRICE (Protection, Rest, Ice, Compression, Elevation) vs. PEACE and LOVE (Protection, Elevation, Avoid anti-inflammatories, Compression, Education and Load, Optimism, Vascularization, Exercise) in adolescent lateral ankle sprain rehabilitation: a randomized prospective comparative study of muscle strength and dynamic balance
Key Points
- Both PRICE plus NSAIDs and PEACE and LOVE resulted in similar improvements in strength, range of motion, and dynamic balance over 12 to 15 weeks in adolescents with lateral ankle sprain.
BACKGROUND & OBJECTIVE
Lateral ankle sprain is one of the most common injuries in adolescent sport and carries a high risk of progressing to chronic ankle instability (1). Despite this, early management strategies remain debated.
The traditional approach, PRICE combined with NSAIDs, has been widely used for decades. However, emerging evidence suggests excessive rest and anti-inflammatory strategies may impair aspects of tissue healing. In contrast, PEACE and LOVE represents a more biologically active and rehabilitation driven strategy, while PRICE primarily targets short term symptom relief. PEACE and LOVE also emphasise education, early optimal loading, and progressive rehabilitation.
This study aimed to compare short term functional recovery between PRICE plus NSAIDs and PEACE and LOVE in adolescents following a first-time lateral ankle sprain.
PEACE and LOVE provide a more active and structured rehabilitation framework; early loading, education, and progressive exercise align with known principles of tissue adaptation and neuromuscular recovery.
METHODS
This prospective randomized study included 76 adolescents aged 12 to 17 years presenting within one to four days of a first-time lateral ankle sprain. Participants were allocated to either PRICE plus NSAIDs or PEACE and LOVE using concealed randomization. 65
